Please indicate (initial or check) authorization for sharing your information (name, email, phone, & address) as desired as well as your willingness to sponsor new members.Memberships applications MUST be signed by the applicant. Unsigned applications will not be processed. |

NOTE: All new members must be sponsored by a member who has been in good standing for two years. New membership applications submitted without sponsorship by a member in good standing will NOT be processed.
